Broker of Record Authorization

Effective Date: August 24, 2026 Last Updated: August 24, 2026

This Broker of Record Authorization ("Authorization") explains what you agree to when you designate the licensed insurance agent assisting you through the Aloe platform as your Agent of Record for the individual health insurance policy you enroll in. Anjoy Collective, Inc. doing business as Aloe Health ("Aloe") operates the platform. Capitalized terms not defined here have the meaning given in our Terms of Service.

By completing enrollment, you provide this Authorization.

1. Appointment as Agent of Record

You authorize the licensed insurance agent assigned to you through the Aloe platform (the "Agent") to act as your agent, broker, and Agent of Record with the insurance carrier for the individual health insurance policy you select. This designation applies to the policy you enroll in through the platform and is subject to the carrier's requirements.

2. What the Agent Does

The Agent may help you compare plans, submit your enrollment, and service your policy, including assisting with questions, changes, and renewals. The Agent must hold an active health insurance producer license in your state and the applicable line of authority.

3. Compensation

Any commission for your individual policy is paid directly by the insurance carrier to the Agent in accordance with the carrier's standard commission schedule. This does not increase your premium and is not an additional charge to you. Carrier commissions are separate from any fees Aloe charges your employer.

4. Aloe's Role

Aloe provides the platform that connects you with the Agent and administers your employer's ICHRA. Aloe is not an insurance company and does not take Broker of Record or Agent of Record designation on your individual policy. Aloe does not collect or distribute carrier commissions.

5. Consent to Share Information

You authorize Aloe and the Agent to share the information reasonably necessary to place and service your policy with the insurance carrier and with each other, consistent with our Privacy Policy and applicable law.

6. Changing Your Agent of Record

You may change your Agent of Record at any time, subject to the carrier's process and requirements. Changing your Agent of Record does not by itself change your coverage.

7. Voluntary Authorization

This Authorization is voluntary and reflects your choice to have the Agent assist you and be designated as your Agent of Record. It remains in effect for your policy until you change your Agent of Record or your policy ends.

8. Governing Law and Contact

This Authorization is governed by the laws of the State of Michigan, without regard to its conflict-of-laws rules. Questions can be sent to hello@aloehealth.co.
